Here are two examples of consent forms for your use. One form is intended for interviewees who are willing to sign the form with their name. The other (oral consent), can be used is the rare cases when the interviewees are not willing to
reveal their identity on a form. In these cases, the interviewer will declare that the consent was given orally and will sign the form.
